Support the facility with the integrated clinical management of clients for TB services 35%
- Implement MECC principles and update 100% of patient folders
- Receive missed appointment lists daily from admin support staff and action it.
- Implement TUTT
- Work in selected high priority facilities
- Quality TB Screening in accordance with guidelines and High-risk HIV clients
- Quality TB testing by collection of sputum samples
- TB Contact tracing and management of active Cases.
- The tracing of disengaged TB clients
- Adherence support to all clients.
- Support integration with other services, chronic diseases, family planning, HIV
- STI screening and referral, treatment
- Linkage of patients to care for NPO’s and mobiles
- Attend facility meetings for feedback
- Attend sub-distrct/District meetings mandatory

- Quality collection of blood samples as per guidelines.
- Correct VL management according to the unsuppressed guideline
- Support integration with other services, chronic diseases, family planning, HIV

- Integration of HIV self-screening /PrEP for partners and refer accordingly.
- Adherence support to all clients
- Linkage of patients to care for NPO’s and mobiles
- Optimal use of the CBS referral app
- Attend facility meetings for feedback
- Attend sub-district/District meetings mandatory.
- Optimal use of SPV
Awareness and health promotion and education within facilities 10%
- Planning for daily health promotion events within facility
- Childhood HIV/TB disease prevention and treatment
- Adult HIV /TB disease prevention and treatment
- Health education on TB Contact management
- Support the Utilization of CHWs for health promotion & education.
- Condom use: demonstration and distribution: male and female-prevention of transmission
- Promote HIV self-screening /PrEP partners and refer.
- Awareness of HIV /TB disease, signs and symptoms, adherence, treatment, self-care
- Healthy eating and WOW promotion
- Encourage to wear face mask
- Monitor that IPC practices are always followed
- Health education and implementation of IPC
- Manage and refer clients with Chronic diseases of lifestyle, as part of integration services.
- Optimal use of the CBS referral app.
- Optimal use of SPV
Administrative duties 10%
- Attend PDC training sessions on-line
- Implement PACK 2025
- Implement Vertical transmission Prevention
- TB prevention & Management
- HIV Testing services
- HIV prevention & management
- IQC
- Support the NPO assigned admin/intern staff
- Linkage with the NPO OTL
- Attend all relevant meetings Implement ART guidelines if authorized or refer clients immediately
Information management 10%
- Establish work relationship with facility info clerks to support
- Complete necessary data collection tools daily,
- Completion of NHLS documents & Clinical stationery
- Validate data before submission
- Report incidents
- Report adverse events
- Feedback to OPM/project manager/SD clinical coordinator
- Assist Folder flow to data capturer
- Ensure and oversee daily capturing of clients
- Grade 12
- Diploma in Nursing (General, Midwifery, Community Science, Psychiatry)
- Registration as a professional Nurse with the South African Nursing Council.
- HIV Management Course
- NIMART trained
- Trained in Adult PACK Experience working in primary healthcare facilities in the public sector
- Minimum 3 years’ experience managing HIV-positive patients
- Drivers Licence and willingness to use own vehicle
Requirements
- Grade 12
- Diploma in Nursing (General, Midwifery, Community Science, Psychiatry)
- Registration as a professional Nurse with the South African Nursing Council
- HIV Management Course
- NIMART trained
- Trained in Adult PACK
- Experience working in primary healthcare facilities in the public sector
- Minimum 3 years’ experience managing HIV-positive patients
