Purpose of the position:

  • The Linkage officer will be responsible for the successful navigation and linkage of AYP to various services in the AYP Programme. 

Minimum Requirements    

  • Grade 12) or equivalent to NQF 4
  • HTS counselling certificate will be advantageous
  • Experience in linkage to care but not requirement
  • Worked with / in primary health care facilities and/or hospital setting preferable
  • Worked with AYP in a community setting
  • Knowledge of key populations programming preferable
  • Knowledge of HIV, STIs and TB prevention approaches
  • Ability to work non-traditional/flexible hours.
  • Ability to travel as and when required
  • MS Office proficiency
  • Criminal clear

Duties and Responsibilities    

  • When on outreach, support peer educators by recruiting AGYW/YWKP clients to the nurse or Health and Wellness Counsellor (HWC) for services
  • Able to assist with HIVSS self-screening as per protocol
  • Develop strong relationships with health facilities in the subdistrict to enable successful linkage
  • Feedback to the NiMART Professional Nurse (PN) regarding referrals
  • Support PN to trace partners of positive index clients if permission was granted by the index client
  • Navigate AGYW/YWKP into health facility to ensure linkage to care
  • Data collection on referrals and successful linkage to care
  • Completing all required reports for the programme on linkage and referrals
  • Maintain a system of tracking newly diagnosed HIV, TB and STI clients telephonically or home visits
  • Monitoring linkage data and ensuring targets are met
  • Attend meetings, briefings, debriefing, staff meetings and other as directed
  • Understand and advocate for AGYW/YWKP rights to health care services
